LTSS Service Care Manager - Behavioral Health

Location: Hybrid role based in Texas, serving Tyler, TX and surrounding counties; frequent home and site visits are required. Work schedule is Monday-Friday, 8:00 am-5:00 pm CST, with no on-call, evening, weekend, or holiday work.

Salary: $27.02-$48.55 per hour

What you will do

• Assess complex, high-risk members with mental and behavioral health needs and develop individualized care or service plans.
• Coordinate care among members, families, physicians, facilities, agencies, specialists, and community providers.
• Conduct frequent home and site visits to assess needs, collaborate with resources, and support member engagement.
• Educate members and caregivers on treatment options, medications, preventive care, healthcare benefits, and available services.
• Coordinate medical, behavioral, and social resources, including housing, transportation, meals, foster care, and activities of daily living.
• Document care management activities, support service authorizations, and promote compliance with clinical, state, and federal guidelines.

Requirements

• Master’s degree in Mental Health Counseling or Social Work.
• 2-4 years of related experience; 4+ years of case management experience is preferred.
• Unrestricted behavioral health professional license required in the applicable state, such as LCSW, LMFT, LMHC, or LPC.
• Experience with behavioral health populations, complex caseloads, clinical documentation, problem-solving, and care coordination.
• Ability to work independently, manage changing priorities, and use web-based applications and case management systems.
• DFPS background check required for Superior Health Plan.

Nice to have

• Experience in child welfare, adoption, foster care, Child Protective Services, pediatric acute care, inpatient treatment, schools, or public mental health agencies.
• Experience supporting Foster Care Medicaid recipients and working in managed care or government-sponsored programs.
• Field-based experience.
• Bilingual English/Spanish skills.
