RN, Field Care Manager (Maternal-Child Health)

Location: Remote home office with primarily field-based work in Regions 1, 3, or 9 of Southern Louisiana; candidates must reside in the specified Louisiana parishes. Travel up to 50% within the assigned Louisiana region is required for member, family, provider, community, and team interactions.

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Assess members' physical, environmental, and psychosocial needs through telephonic and in-person interactions.
• Develop and update individualized care plans based on clinical judgment and changes in health status.
• Identify barriers to effective care and coordinate physical health, social determinant, and additional benefit services.
• Collaborate with primary care providers, care teams, community agencies, and families to support quality and cost-effective outcomes.
• Monitor member progress, provide health and wellness resources, and facilitate care transitions.
• Participate in health promotion, community events, incident reporting, and activities required by Louisiana Department of Health guidelines.

Requirements

• Must reside in Region 1, 3, or 9 of Southern Louisiana, including the listed eligible parishes.
• Active, unrestricted Louisiana Registered Nurse license.
• At least two years of clinical experience in maternal-child health, Labor & Delivery, and/or pediatrics.
• Intermediate to advanced computer skills, including Microsoft Word, Outlook, Excel, and electronic medical records.
• Strong communication and interpersonal skills with the ability to build rapport quickly.
• Ability to work independently, exercise sound judgment, and adapt care plans to changing member needs.

Nice to have

• Case Management Certification or willingness to obtain it within two years.
• Field case management, managed care health plan, Medicare, Medicaid, or dual-eligible population experience.
• Knowledge of maternal-child mortality and morbidity, breastfeeding, community resources, health promotion, coaching, and wellness.
• Motivational interviewing experience.
