Registered Nurse Care Manager (Healthcare)

Location: Hayward/Spooner area, Wisconsin, United States. Remote from home with travel of up to 40% for in-person member visits and occasional travel to hirify.global offices for training or meetings. Reside within 45 minutes of the assigned coverage area is preferred. Monday through Friday, 8:00 am–4:30 pm CST.

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Assess members’ health and safety needs and develop comprehensive Member Care Plans.
• Conduct regular health assessments and coordinate care with acute and primary care providers.
• Educate members about disease processes, risks, nutrition, exercise, and lifestyle modifications.
• Conduct monthly phone contacts and travel to members’ settings for in-person visits.
• Review and remediate member records, documenting updates within required DHS contract timeframes.
• Support newly assigned RNs and Care Coach partners, assign follow-up tasks in D365, and escalate significant risks or concerns.

Requirements

• Associate degree in nursing.
• Licensed Registered Nurse in the state of Wisconsin and in good standing.
• Valid state driver’s license and personal vehicle liability insurance with minimum limits of 100/300/100.
• Patient-facing work requires participation in hirify.global’s tuberculosis screening program.
• Dedicated home workspace and internet service with at least 25 Mbps download and 10 Mbps upload speeds.

• One year of care management experience.
• Previous care management experience.
