Case Manager (CCM) (Healthcare)

Location: Remote within Arizona, Florida, Georgia, Illinois, Iowa, Kansas, Michigan, Missouri, Nebraska, New Jersey, North Carolina, Ohio, Pennsylvania, South Carolina, Tennessee, Texas, or Virginia. Occasional travel may be required for team meetings and company events.

Salary: $75,348–$98,894 per year in California, Connecticut, New Jersey, New York, and Washington; $67,813–$89,005 per year in other locations.

What you will do

• Coordinate care across inpatient, outpatient, emergency, and home-care settings.
• Partner with members and providers to create personalized care plans and improve clinical outcomes.
• Assess Social Determinants of Health and connect members with appropriate community resources.
• Provide crisis intervention and advocate for members by resolving barriers to care and treatment compliance.
• Educate members by phone, secure messaging, or in person about health conditions, hospital discharges, medical procedures, and prescriptions.
• Contribute to process improvement initiatives and pilot programs while maintaining confidentiality and regulatory compliance.

Requirements

• At least 2 years of clinical experience in hospital, outpatient, or community-based care management.
• At least 2 years of experience working directly with patients with chronic medical conditions such as diabetes, COPD, or hypertension.
• At least 1 year of experience in care coordination and navigation.
• Active, unrestricted US RN license in the relevant state, an active compact multistate unrestricted RN license, or an MSW from a nationally accredited school approved by the Council on Social Work Education.

Nice to have

• BSN.
• Licensed Master Social Worker or Licensed Clinical Social Worker credential.
• CCM or Complex Case Management certification.
• Case management experience.
• Bilingual Spanish skills.
