What you will do
- Provide comprehensive medication management for patients with chronic and complex conditions.
- Evaluate, initiate, modify, monitor, and discontinue medications under applicable regulations and collaborative practice agreements.
- Conduct patient assessments and medication reviews, identifying drug therapy problems and opportunities to improve outcomes.
- Monitor therapeutic effectiveness, adverse drug reactions, adherence, and patient safety.
- Collaborate with physicians, advanced practice providers, nurses, and interdisciplinary care teams on individualized treatment plans.
- Provide patient education, document clinical interventions in the EHR, and support quality improvement and population health programs.
Requirements
- Doctor of Pharmacy (PharmD) degree from an accredited pharmacy school.
- Current and unrestricted pharmacist license in the applicable state.
- Advanced Practice Pharmacist designation or eligibility under California regulations.
- At least two years of clinical pharmacy experience in ambulatory care, primary care, managed care, or a related setting.
- Strong knowledge of chronic disease management and evidence-based treatment guidelines.
- Experience with electronic health records, clinical documentation, communication, collaboration, and patient education.
Nice to have
- Completion of a PGY1 pharmacy practice residency or equivalent clinical experience.
