Case Manager (Healthcare)

Location: Remote position supporting members in Taylor/Price Counties and surrounding areas, Wisconsin. Travel up to 40% is required for in-person member assessments and visits, with occasional travel to hirify.global offices for training or meetings. Typical hours are Monday–Friday, 8:00 a.m.–4:30 p.m. CST.

Salary: $53,700–$72,600 per year

What you will do

• Assess members’ health and safety needs, strengths, interests, and preferences.
• Develop and maintain comprehensive Member Care Plans.
• Coordinate with Field Care Nurses and arrange services in the least restrictive environment.
• Conduct enrollment assessments, quarterly home visits, and monthly telephone contacts.
• Arrange social integration, community, employment, housing, and other non-medical support services.
• Evaluate risks, provide education, manage service authorizations, and maintain case documentation.

Requirements

• Bachelor’s degree in human services or a related field plus at least one year of experience serving frail elders or adults with intellectual, developmental, or physical disabilities; alternatively, a bachelor’s degree in another field plus at least three years of relevant experience.
• Ability to travel up to 40% and visit members in Taylor/Price Counties and surrounding areas.
• Valid state driver’s license and personal vehicle liability insurance with minimum 100/300/100 limits.
• Ability to work Monday–Friday during 8:00 a.m.–4:30 p.m. CST hours.
• Patient-facing role requiring participation in hirify.global’s TB screening program.
• Dedicated home workspace and internet service with at least 25 Mbps download and 10 Mbps upload speeds to protect member PHI and HIPAA information.

Nice to have

• Residence within 45 minutes of the assigned coverage area.
• Case management experience.
• Experience with electronic case-note documentation.
• Knowledge of community health, social service agencies, and community resources.
