Registered Nurse Care Manager (Wisconsin Family Care)

Location: Remote Wisconsin; assigned coverage area near Ashland, WI. Travel up to 40% for in-person member visits and occasional travel to hirify.global offices for training or meetings. Work schedule: Monday–Friday, 8:00 a.m.–4:30 p.m. CST.

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Assess members’ health and safety needs and develop comprehensive Member Care Plans.
• Conduct regular health assessments and coordinate with acute and primary care providers.
• Educate members about disease processes, nutrition, exercise, lifestyle changes, and related risks.
• Document member information within required DHS contract timeframes and participate in monthly phone contacts.
• Review records for reassigned members, identify remediation needs, and complete required updates.
• Support newly assigned RNs and Care Coach partners, assign follow-up tasks in D365, and escalate significant risks or concerns.

Requirements

• Associate degree in nursing.
• Registered Nurse license in the state of Wisconsin in good standing.
• Valid state driver’s license and personal vehicle liability insurance with minimum 100/300/100 limits.
• Ability to travel up to 40% for member visits.
• Ability to work from a dedicated home workspace with internet speeds of at least 25 Mbps download and 10 Mbps upload.
• Patient-facing work requires completion of TB screening if selected.

Nice to have

• At least one year of care management experience.
• Residence within 45 minutes of the assigned coverage area.
• Previous care management experience.
