Government Programs Care Manager Support Specialist II (Healthcare)

Location: Remote work from home; candidate must reside in the Las Cruces, New Mexico area. The posting also lists NM - Albuquerque.

Base pay: $18.46–$37.84 per hour.

What you will do

• Contact members and community partners to support healthcare education, quality campaigns, and care initiatives.
• Assist with scheduling care manager and member appointments.
• Support members with health-related social needs through social-service departments and community-based organizations.
• Respond to member inquiries and support physical and behavioral health clinicians with provider and member activities.
• Maintain relationships with community partners and contribute to community health outreach programs.
• Conduct outreach screenings, assessments, education, and inquiries for low-risk members; field-based work may be required for campaigns and departmental activities.

Requirements

• Bachelor’s degree, or one year of college plus three years of health, social-service, or community experience, or four years of relevant experience.
• Knowledge of medical terminology and experience coordinating member needs, assisting members, and analyzing needs.
• Proficiency with Microsoft Office and strong customer-service, verbal, and written communication skills.
• Experience with coaching and motivational interviewing to educate members about medical and behavioral-health issues.
• New Mexico applicants must have a bachelor’s degree in Social Work, proficiency in a New Mexico Native American Pueblo language with the ability to translate and present, and Community Health Worker certification.
• Residence in the Las Cruces, New Mexico area is required.
