Community Health Worker - Medicaid

Location: Remote, work from home in Pennsylvania, United States; 50%–75% travel required for community outreach and events.

What you will do

• Conduct face-to-face and telephone outreach focused on preventive health, care gaps, maternity, care transitions, chronic conditions, and health-related social needs.
• Educate members, provide basic health-literacy support, and help them navigate healthcare services and benefits.
• Assess social needs, coordinate referrals, schedule appointments, and connect members with community resources and providers.
• Build partnerships with organizations including Head Start programs, WIC, community centers, homeless shelters, and healthcare providers.
• Prepare educational materials, document outreach outcomes, and contribute to measurable programs for at-risk members.
• Represent hirify.global at community events and collaborate with care management and internal teams.

Requirements

• At least one year of experience in healthcare customer service, marketing, community health work, or a related field.
• High School diploma or GED.
• Ability to provide empathetic, culturally sensitive support to Medicaid members, including members with limited English proficiency.
• Strong written and oral communication, customer service, independent problem-solving, organization, and multitasking skills.
• Certified Community Health Worker certification from the Pennsylvania Certification Board required within 12 months of the start date.
• Must work remotely from Pennsylvania and be able to travel 50%–75% for community-based duties.

• Basic medical terminology knowledge.
• Experience with the target community or population.
