Community Health Worker (Healthcare)

Location: Remote work in Pennsylvania, United States, with face-to-face community outreach across the Allegheny County territory. Travel requirement: 50–75%.

What you will do

• Conduct face-to-face outreach for preventive health, maternity, care transition, and chronic condition initiatives.
• Educate members, complete Health Risk Assessments, identify healthcare barriers, and help schedule appointments.
• Coordinate referrals and access to providers, care management programs, behavioral health services, and community agencies.
• Build supportive relationships with members, community centers, human services providers, and state agencies.
• Develop educational materials and conduct provider and community training on EPSDT and preventive health services.
• Represent hirify.global at community events and report training outcomes and program results.

Requirements

• High school diploma or GED.
• At least one year of experience in a healthcare-related customer service or marketing environment.
• Customer service, communication, problem-solving, organization, patience, and empathy skills.
• Ability to work independently with Medicaid members and address transportation, childcare, financial, and healthcare access barriers.
• Ability to travel 50–75% and work directly with members and community organizations in Pennsylvania.
• Understanding of cultural diversity and sensitivity when supporting members with limited English proficiency.

Nice to have

• Case management experience.
• Community Health Worker experience.
• Experience in the nonprofit sector or with the target community or population.
• Basic medical terminology knowledge.
