CCM Case Manager (Healthcare)

Location: Remote within the United States; candidates must reside in Arizona, Florida, Georgia, Illinois, Iowa, Kansas, Michigan, Missouri, Nebraska, New Jersey, North Carolina, Ohio, Oklahoma, Pennsylvania, South Carolina, Tennessee, Texas, or Virginia. Occasional travel may be required for team meetings and company events.

Salary: $75,348–$98,894 per year in California, Connecticut, New Jersey, New York, and Washington; $67,813–$89,005 per year in other eligible locations.

What you will do

• Coordinate care across inpatient, outpatient, emergency, and home-care settings while maintaining confidentiality and professional standards.
• Partner with members and providers to develop personalized care plans for complex health challenges.
• Assess Social Determinants of Health and connect members with appropriate community resources.
• Provide crisis intervention and advocate for members by resolving barriers to care and treatment compliance.
• Educate members by phone, secure messaging, or in person about health conditions, hospital discharges, medical procedures, and prescriptions.
• Support process improvement initiatives and new pilot programs while complying with applicable laws and regulations.

Requirements

• At least 2 years of clinical experience in hospital, outpatient, or community-based care management.
• At least 2 years of experience working directly with patients with chronic medical conditions such as diabetes, COPD, or hypertension.
• At least 1 year of experience in care coordination and navigation.
• Active, unrestricted U.S. RN license in the applicable state, an active compact multistate unrestricted RN license, or an MSW from a nationally accredited program approved by the Council on Social Work Education.

Nice to have

• BSN.
• Licensed Master Social Worker or Licensed Clinical Social Worker credentials.
• CCM or complex case management certification.
• Case management experience.
• Bilingual Spanish skills.
