RN, Senior LTSS Service Care Manager (Pediatric Care)

Location: Remote in Texas with required travel for member assessments near Katy, Richmond, Rosenberg, and Sugar Land. Standard schedule is Monday–Friday, 8:00 AM–5:00 PM CST, with flexibility for pediatric members' school schedules. Home or other site visits may occur once a month or more.

Salary: $36.21–$65.09 per hour

What you will do

• Assess complex, high-risk, and high-acuity pediatric members and develop personalized long-term care and service plans.
• Coordinate providers, specialists, caregivers, facilities, agencies, and community resources to deliver person-centered services.
• Monitor member conditions, clinical symptoms, care-plan progress, complications, and potential needs for higher levels of care or waiver eligibility.
• Educate members and families on disease processes, care options, referrals, benefits, provider instructions, and available community resources.
• Coordinate service authorizations for services such as transportation, housing, meals, foster care, employment, and activities of daily living.
• Document care-management activities, support regulatory compliance, improve quality, and may precept and coach new clinical hires.

Requirements

• Graduate from an accredited school of nursing and hold RN qualifications.
• 4–6 years of related experience, including clinical nursing, RN case management, or service coordination.
• Experience with pediatric acute care, pediatric home health, or pediatric private-duty nursing is preferred.
• Experience in home health, hospice, long-term care, or managed care is valued.
• Must be located in Texas and able to travel for member assessment visits.
• Bachelor of Nursing degree is preferred.
