Care Manager (RN)

Location: Remote within Mississippi; must reside in Mississippi and hold an active Mississippi RN license.

Salary: $56,200–$101,000 per year

What you will do

• Assess member needs, access barriers, social determinants of health, and member-defined priorities.
• Develop and update personalized care plans and service plans.
• Coordinate with members, families, caregivers, providers, specialists, and community resources.
• Monitor member status, changes in condition, progress, and care plan outcomes.
• Provide education on disease processes, care gaps, provider instructions, care options, referrals, and healthcare benefits.
• Document care management activities and provide feedback on opportunities to improve care quality and cost-effectiveness.

Requirements

• Must live in Mississippi.
• Active Registered Nurse license in Mississippi or a compact state license is required.
• Degree from an accredited school of nursing or a bachelor's degree in nursing.
• 2–4 years of related experience.
• Experience with assessment and case management is preferred.
