Care Manager (RN)

Location: Hybrid role for candidates residing in Delaware, with in-person member visits approximately two days per week and the remainder performed remotely.

Salary: $56,200–$101,000 per year.

What you will do

• Assess member needs, barriers to care, and social determinants of health, then develop personalized care and service plans.
• Coordinate with members, families, caregivers, providers, specialists, and community resources to address unmet needs.
• Monitor member status, changes in condition, progress toward goals, and update care plans as needed.
• Provide telephonic, digital, home, and other site outreach, including education about diseases, care options, referrals, benefits, and provider instructions.
• Document member information and care management activities in compliance with applicable regulatory and payer requirements.
• Identify opportunities to improve care quality and delivery while supporting cost-effective outcomes.

Requirements

• Must reside in Delaware.
• Active, unrestricted Delaware RN license is required.
• Degree from an accredited school of nursing or a Bachelor’s degree in Nursing.
• 2–4 years of related experience.
• RN state licensure and/or compact state licensure.
• Ability to coordinate complex care management activities and collaborate with members, caregivers, providers, and community resources.

Nice to have

• Experience supporting high-risk maternity populations.
• Previous case management experience.
