Care Coordinator (LTSS)

Location: Remote/field-based in Illinois, with 75–90% local travel to members’ homes, assisted living facilities, long-term care facilities, and other care settings. Occasional onsite meetings may be required at the hirify.global office in Schaumburg, IL. Work schedule: Monday–Friday, 8:00 AM–5:00 PM CST, with flexibility based on business needs.

Salary: $53,700–$72,600 per year.

What you will do

• Assess Medicaid members’ health, care, and long-term support needs.
• Coordinate services and develop, implement, and monitor individualized care plans.
• Identify and resolve barriers that affect effective care delivery.
• Guide members and families toward appropriate healthcare, community, payer, and provider resources.
• Monitor progress toward member goals through assessments, data, conversations, and active care planning.
• Collaborate with providers, interdisciplinary teams, community partners, and other stakeholders.

Requirements

• Bachelor’s degree in social sciences, social work, human services, or a related field, or an active unrestricted Illinois LPN license with at least one year of relevant assessment and formal-services experience.
• At least one year of experience in healthcare and case management.
• At least one year of experience with Medicare and Medicaid recipients, long-term care services, HCBS, and managed care organizations.
• Intermediate to advanced computer skills, including Microsoft Word, Excel, Outlook, and electronic medical records.
• Ability to travel throughout the region for face-to-face member, family, community partner, and care-team interactions.
• Valid state driver’s license and personal vehicle liability insurance are required; TB screening is required for selected candidates.

Nice to have

• Active unrestricted Illinois LPC or LSW license.
• Bilingual or multilingual ability in English and Spanish, Arabic, Vietnamese, Amharic, Urdu, or another language.
