Care Manager Transition of Care (RN)

Location: Remote in California; California residency is preferred and an active California RN license is required

Salary: $27.02–$48.55 per hour

What you will do

• Complete post-discharge assessments for members transitioning from healthcare facilities.
• Review and reconcile pre-admission and post-discharge medications.
• Develop care and service plans in collaboration with discharge planners, providers, specialists, and interdisciplinary teams.
• Assess health status, treatment plans, services, and socioeconomic resource needs, then provide appropriate interventions.
• Facilitate transitions into active care management and coordinate medical and non-medical services.
• Provide education, resource materials, outreach, documentation, and feedback to improve care quality and compliance.

Requirements

• Active RN license in California is required.
• A degree from an accredited school of nursing, or a master's degree in Behavioral Health or Social Work.
• Two to four years of related experience.
• RN, LISW, LCSW, LMSW, LMFT, LMHC, or LPC certification is required.
• Experience with discharge planning, care coordination, or postpartum care is strongly preferred.
