Family Care-Registered Nurse (RN Case Manager)

Location: Field-based role within specified Wisconsin counties and locations, including Milwaukee, Kenosha, La Crosse, Madison-area communities, Racine, Sheboygan, Waukesha, and others. Candidates must live within commuting distance of an office and travel to member homes and community locations; occasional office attendance is required.

What you will do

• Perform telephonic and face-to-face functional assessments covering physical health, behavioral health, social services, and long-term supports.
• Manage care for members with chronic illnesses, co-morbidities, and disabilities.
• Develop, monitor, evaluate, and revise individualized, person-centered care plans.
• Set short- and long-term goals with members, caregivers, families, physicians, and natural supports.
• Coordinate service authorizations, referrals, alternative levels of care, and waiver programs.
• Collaborate with Medical Directors, Physician Advisors, interdisciplinary teams, providers, claims teams, and service partners.

Requirements

• Associate of Science in Nursing or Bachelor of Science in Nursing, or an equivalent combination of education and experience.
• At least three years of experience working with individuals with chronic illnesses, co-morbidities, or disabilities in service coordination, case management, or a similar role.
• Current, active, valid, and unrestricted Wisconsin RN license required.
• At least one year of experience working with a Family Care target population.
• Strong interpersonal, communication, collaboration, problem-solving, and crisis-management skills.
• Ability to travel to worksites, member homes, communities, and other designated locations.

Nice to have

• BA or BS in Health or Nursing.
• Case management experience with older adults or people with disabilities.
• Long-term care or Family Care/Partnership experience.
• Comfort using technology and commitment to culturally responsive, member-centered care.
