Nurse Practitioner (Home-Based Care)

Location: Hybrid field and telehealth role covering Essex County, New Jersey; travel throughout the assigned service area is required.

Salary: $155,000–$165,000 USD annually.

What you will do

• Conduct in-home and telehealth visits for Medicare Advantage members.
• Perform comprehensive assessments, medication reviews, and chronic disease management.
• Develop individualized care plans addressing clinical needs and social determinants of health.
• Identify fall risks, food insecurity, caregiver challenges, and treatment-adherence gaps.
• Collaborate with physicians, care coordinators, social workers, and other interdisciplinary professionals.
• Use Clover Assistant, EMR, and telehealth technology to support proactive care and prevent avoidable hospitalizations.

Requirements

• Active Nurse Practitioner license in New Jersey or the ability to obtain it.
• National board certification as a Nurse Practitioner.
• Active DEA and New Jersey CDS registration, or the ability to obtain them.
• Experience caring for adult or geriatric populations in primary care, complex care, chronic care management, transitional care, home-based care, or value-based care.
• Strong clinical assessment, chronic disease management, communication, and collaboration skills.
• Ability to use technology and telehealth platforms to support patient care.

Nice to have

• Experience in home care, home health, home-based primary care, palliative care, geriatrics, or population health.
• Experience with Medicare Advantage, value-based care, or medically complex and high-risk populations.
• Bilingual skills.
