Nurse Practitioner (1099 / PRN) – In-Home Assessments

Location: On-site in Twin Falls, Idaho, United States, with daily travel within a 55-mile radius of the assigned county.

Pay: Approximately $120 per completed assessment plus paid mileage.

What you will do

• Conduct in-home Annual Wellness Visits and chronic condition assessments.
• Perform patient histories, physical examinations, vital-sign checks, medication reviews, and preventive screenings.
• Complete Hemoglobin A1C, spirometry, KED, DRE, and FIT screenings.
• Educate members on preventive care and chronic disease management.
• Document visits in the EMR to support value-based care and risk adjustment.
• Work independently in the field while honoring confirmed visits and assignments.

Requirements

• Active, unencumbered Nurse Practitioner license in the applicable state.
• Three or more years of patient-care experience preferred.
• Comfort with EMR systems and independent work.
• Availability for approximately 30 hours per month on an ongoing basis.
• Ability to travel daily within a 55-mile radius of the assigned county.
• Willingness to obtain additional licensure if needed, with support from hirify.global.
