Care Navigator (Foster Care)

Location: Remote from anywhere within the state of Missouri; may include on-site visits to assess member needs and collaborate with providers. Work schedule: Monday–Friday, 8am–5pm.

Salary: $22.94–$38.79 per hour

What you will do

• Assess the needs of foster care members, barriers to care, available resources, and appropriate support plans.
• Develop and maintain personalized care plans and identify providers, specialists, and community resources.
• Coordinate care between members, families, caregivers, healthcare providers, and care management teams.
• Provide psychosocial support and education about healthcare services, benefits, referrals, procedures, and care options.
• Monitor progress toward care-plan goals, recommend revisions, and refer members for further evaluation when needed.
• Document member information and care management activities in compliance with regulatory and payer requirements.

Requirements

• Bachelor’s degree and 2–4 years of related experience, or equivalent applicable knowledge and experience.
• Previous case management or foster care experience is strongly preferred.
• Graduate from an accredited school of nursing if holding clinical licensure.
• Current Missouri clinical license, including LPN licensure, is preferred.
• Must work remotely from within the state of Missouri.
