Field Care Coordinator (Bilingual)

Location: Remote position for candidates residing in Homestead or southern Miami-Dade County, Florida; 75–90% local travel to members’ homes, assisted living facilities, long-term care facilities, and other care settings. Occasional travel to hirify.global offices for training or meetings may be required.

Salary: $62,000–$66,000 per year based on experience and qualifications. The broader estimated base pay range is $53,700–$72,600 per year.

What you will do

• Visit Medicaid members in homes, assisted living facilities, long-term care facilities, and other care settings.
• Assess member needs, barriers, and requirements and establish individualized care plans.
• Plan and implement interventions that support member goals and optimal well-being.
• Coordinate services with providers, payer sources, interdisciplinary teams, families, and community resources.
• Monitor and evaluate case management plans and connect members with appropriate services.
• Provide health education, encouragement, and wellness support.

Requirements

• Bachelor’s degree in Health Services, Social Work, Psychology, or a related field with at least 2 years of relevant experience; or Florida-licensed RN with at least 2 years of relevant experience; or Florida-licensed LPN with at least 4 years of relevant experience; or at least 6 years of health services or case management experience as a case worker.
• Prior experience with Medicare and Medicaid recipients.
• Intermediate to advanced computer skills and experience with Microsoft Office.
• Valid state driver’s license and proof of personal vehicle liability insurance required.
• AHCA fingerprinting, Florida Level 2 background screening, and tuberculosis screening required.
• Availability Monday through Friday, 8:30 a.m. to 5:00 p.m.; scheduled for 40 hours per week.

Nice to have

• Bilingual English and Spanish skills with the ability to pass a language proficiency exam.
• Nursing home diversion or long-term care case management experience.
• Experience with electronic case notes, geriatric populations, health promotion, coaching, and wellness.
• Knowledge of community health, social service agencies, and community resources.
• 701B certification or medication application experience.
