Care Coordinator Doula

Location: Remote Illinois; must reside in Illinois. Up to 25% local travel is required for member visits, collaboration, and face-to-face meetings. Core hours are Monday–Friday, 8:00 AM–5:00 PM CST, within operating hours of 8:00 AM–7:00 PM CST.

Salary: $53,700–$72,600 per year

What you will do

• Assess the care needs of pregnant and postpartum Medicaid members through telephonic and face-to-face interactions.
• Develop, implement, monitor, and evaluate individualized case management and care coordination plans.
• Identify and resolve barriers to perinatal and postpartum care, including immediate gaps in services and access.
• Coordinate support among members, families, providers, payer sources, social services, and interdisciplinary teams.
• Provide information about managed care requirements, benefits, services, referrals, and community resources.
• Visit Medicaid members in their residences and participate in local field interactions, with travel up to 25%.

Requirements

• Illinois Medicaid-certified doula.
• Must reside in Illinois and be able to complete local travel.
• At least 1 year of healthcare experience and 1 year of case management or care coordination experience.
• Intermediate to advanced computer skills with Microsoft Word, Excel, Outlook, and electronic medical record systems.
• Valid state driver's license and personal vehicle liability insurance meeting state requirements.
• Ability to complete tuberculosis screening and protect member PHI/HIPAA information while working from a dedicated home workspace.

Nice to have

• Experience working with Medicaid recipients and providing doula services during pregnancy, childbirth, and postpartum periods.
• Knowledge of community health, social service agencies, and community resources.
• Previous managed care experience.
• Bilingual ability.
