Field Care Manager, Behavioral Health

Location: Remote, but applicants must reside in Cook or Kane County, Illinois, within the specified ZIP codes or within a 10-mile radius; travel up to 25% within Illinois is required. Work hours are Monday–Friday, 8:00 a.m.–5:00 p.m. CST.

Salary: $71,500–$97,500 per year, plus eligibility for a bonus incentive plan.

What you will do

• Provide integrated behavioral health care management through personalized coordination, crisis intervention, peer support, and provider collaboration.
• Support children, adolescents, adults with serious mental illness or serious emotional disturbance, people with substance use disorders, and justice-involved members.
• Engage members in their communities through face-to-face assessments and telephonic support.
• Coordinate medical and behavioral health services, treatment plans, transitions of care, and service delivery.
• Lead interdisciplinary care team meetings and collaborate with providers, service coordinators, internal departments, and community organizations.
• Address health-related social needs, improve health literacy, and maintain compliance with IDHS, CMS, and NCQA requirements.

Requirements

• Residence in the specified Cook or Kane County, Illinois ZIP codes or within a 10-mile radius is required.
• Active Illinois LCSW, LMFT, or LCPC license; provisional licenses and supervisees are not accepted.
• At least 2 years of post-degree clinical experience in a behavioral health setting.
• Case management experience with complex SMI, SUD, and SED populations.
• Ability to travel to region-based facilities and homes for face-to-face assessments; valid driver’s license, car insurance, and reliable transportation required.
• Experience with electronic medical records and Microsoft Word, Outlook, and Excel, with intermediate to advanced computer skills.

Nice to have

• Case Management Certification (CCM).
• Experience with children, adolescents, foster care, transition of care, or discharge planning.
• At least 3 years of in-home assessment or care coordination experience.
• Experience with Medicare, Medicaid, dual-eligible, managed care, or field case management populations.
• Bilingual ability and knowledge of community health and social service agencies.
