Care Manager, Telephonic Nurse

Location: Remote nationwide within the United States; candidates living in Puerto Rico cannot be hired. Work hours must fit an 8-hour shift between 8:00 a.m. and 6:00 p.m. EST. Mandatory training during the first four weeks runs from 8:00 a.m. to 5:00 p.m. EST. Occasional travel to hirify.global offices for training or meetings may be required.

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Provide comprehensive telephonic case management for complex beneficiaries across the continuum of care.
• Deliver specialized interventions to reduce hospital readmissions and improve appropriate utilization of care sites and funding sources.
• Coordinate integrated care plans addressing beneficiary needs and clinical conditions.
• Collaborate with care management programs, military treatment facility utilization and case management staff, physicians, and other providers.
• Serve as the primary point of contact for activities within the complex case management program.

Requirements

• U.S. citizenship is required under the Department of Defense contract.
• Interim approval for a government security clearance through NBIS must be obtained.
• Active RN license in the state of employment is required.
• At least two years of case management experience and three years of clinical RN or managed care RN experience.
• Ability to obtain CCM certification within one year of hire.
• Dedicated home workspace and internet service with at least 25 Mbps download and 10 Mbps upload speeds.

• Bachelor's degree in nursing.
