Care Coordinator (CISC)

Location: Remote, Albuquerque, New Mexico 87109, United States

Salary: $50,225–$75,335 per year, with actual pay varying by location, skills, experience, education, and other permitted factors.

What you will do

• Coordinate intensive care for members with behavioral health conditions and complex clinical, social, community, and long-term care needs.
• Conduct comprehensive health risk and needs assessments covering psychosocial, physical, medical, behavioral, environmental, and financial factors.
• Develop, communicate, implement, and monitor individualized care plans, including transitions to home care and community-based services.
• Identify gaps in care, evaluate intervention effectiveness, collect variance data, and adjust plans with the interdisciplinary care team.
• Educate members, families, providers, and support staff on care coordination and health strategies.
• Maintain relationships with providers, hospitals, community resources, and other external stakeholders while generating care coordination reports.

Requirements

• 3–5 years of experience in social work, nursing, healthcare, or a related field; relevant experience may substitute for a degree.
• Experience in utilization management, quality assurance, home or facility care, community health, long-term care, or occupational health.
• Knowledge of referral coordination, community and public/private resources, cost-effective care coordination, data interpretation, and trend analysis.
• Ability to analyze complex situations, investigate issues, develop original solutions, and make decisions beyond existing policies or protocols.
• Strong verbal and written communication skills, with the ability to work with clinicians, hospital officials, service agencies, members, and families.
• Valid in-state driver’s license required.

Nice to have

• Associate or bachelor’s degree.
• Certified Case Manager certification.
• Licensed Clinical Social Worker or Registered Nurse licensure.
