Clinical Care Coordinator (Behavioral Health)

Location: On-site in Washington, DC, USA

Salary: $36 per hour plus bonus structure; pay varies depending on credentials and education.

What you will do

• Coordinate whole-person care across physical health, behavioral health, social services, and external healthcare providers.
• Facilitate intakes, readmissions, reassessments, referrals, and transitions to appropriate levels of care.
• Develop, implement, and update treatment plans while monitoring patient progress and maintaining complete records.
• Manage a patient caseload, provide supportive follow-up and crisis intervention, and document all patient contacts.
• Participate in treatment team meetings, outreach activities, community coordination, and clinical program implementation.
• Prepare reports and documentation for third-party reimbursement, criminal justice proceedings, emergency services, and other treatment needs.

Requirements

• Current District of Columbia licensure in one or more of the following areas: physician, psychologist, LICSW, LGSW, APRN, RN, LISW, LPC, LMFT, or LGPC.
• Relevant experience in substance abuse treatment, treatment planning, human services, or behavioral healthcare.
• Knowledge of current drug abuse treatment practices, particularly opioid maintenance treatment.
• Knowledge of community resources, social service agencies, third-party payer requirements, and clinical documentation.
• Strong verbal and written communication, judgment, organization, interpersonal, and customer service skills.
• Regular attendance and presence at the assigned Washington, DC work location are required.
