Field Care Manager Behavioral Health

Location: Remote South Carolina; applicants must reside in South Carolina. Field work is mandatory, including onsite visits to members, with travel up to 50% of the time.

Salary: $65,000–$88,600 per year, plus eligibility for a bonus incentive plan.

What you will do

• Assess and evaluate members’ behavioral, physical, environmental, and psychosocial health needs.
• Guide members and families to appropriate care and community resources.
• Manage member care through assessments, evaluations, interventions, and outcome tracking.
• Create member care plans and resolve barriers that hinder care.
• Coordinate interactions with internal partners, members, families, and external resources.
• Work remotely from home while completing onsite member visits and traveling as required.

Requirements

• Active, unrestricted SC LMSW, LPC, or compact RN license required.
• Must reside in South Carolina and be willing to travel.
• At least 2 years of experience as a behavioral health professional.
• Experience with behavioral change, health promotion, coaching, and wellness.
• Proficiency with electronic information systems and Microsoft Office tools, including Word, Outlook, and Excel.
• A separate room with a locked door is required for a private home office.

Nice to have

• Experience supporting patients by telephone.
• Experience in Community Mental Health, crisis response, Assertive Community Treatment, or substance use disorder treatment.
• Experience working with both children and adults.
