RN Embedded Care Coordinator - Gastroenterology - Foundation Medical Partners

Location: Nashua, New Hampshire, United States. Work shift: 8 a.m.–5 p.m., four days per week.

What you will do

• Coordinate care and provide case management for patients with complex gastroenterology needs.
• Collaborate with providers and interdisciplinary teams to create individualized care plans.
• Educate patients about disease management, medications, nutrition, and self-management.
• Coordinate hospital discharges, specialty referrals, follow-up care, and other transitions of care.
• Use population health tools and electronic medical records to identify patients needing additional support.
• Promote patient safety, quality outcomes, continuity of care, and preventive health initiatives.

Requirements

• Associate Degree in Nursing or equivalent from an accredited institution required; Bachelor of Science in Nursing preferred.
• Current New Hampshire Registered Nurse license and BLS/CPR certification required.
• At least two years of professional nursing experience required.
• Strong clinical assessment, communication, and care coordination skills.
• Knowledge of electronic medical records, chronic disease management, and patient-centered care.
• Ability to manage multiple priorities independently and collaborate with patients, families, and care teams.

Nice to have

• Experience in ambulatory care, case management, population health, or care coordination.
• Bachelor of Science in Nursing.
