Care Navigator LPN (Maternal Health)

Location: Fully remote, but candidates must reside in Oregon and hold an active, unrestricted Oregon LPN license. The role may include occasional on-site member visits when appropriate.

Salary: $22.94–$38.79 per hour

What you will do

• Assess member needs, barriers to care, available resources, and appropriate care-management interventions.
• Develop or contribute to personalized care plans and identify providers, specialists, and community resources.
• Coordinate communication between members, families or caregivers, healthcare providers, and care-management teams.
• Provide education on healthcare procedures, provider instructions, care options, referrals, and benefits.
• Monitor care-plan progress and changes in member condition, collaborating on revisions and further evaluations.
• Document member information and care-management activities in compliance with applicable regulatory and payer requirements.

Requirements

• Must reside in Oregon and hold an active, unrestricted Oregon LPN license.
• Maternal health experience is required, including support for prenatal, postpartum, or maternal-care populations.
• Graduation from an accredited school of nursing is required when holding clinical licensure.
• Bachelor’s degree and 2–4 years of related experience are required, or equivalent applicable knowledge and experience.
• Current state clinical license preferred.
