RN, Field Care Manager (Maternal-Child Health)

Location: Remote Louisiana; candidates must reside in Region 6, 7, or 8 of Northern Louisiana, covering the listed parishes. The role is primarily field-based, with up to 50% travel for member, family, provider, community, and team interactions. Remote home-office work is available when not conducting visits.

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Assess members’ physical, environmental, and psychosocial needs through telephonic and in-person interactions.
• Develop and update individualized care plans using clinical judgment and changes in health status.
• Identify and resolve barriers to effective care while connecting members and families with appropriate resources.
• Coordinate physical health services, social determinants of health support, additional benefits, and care transitions.
• Collaborate with primary care providers, other care team members, community services, and families.
• Participate in health promotion, community events, incident reporting, and care management activities under Louisiana Department of Health requirements.

Requirements

• Must reside in Region 6, 7, or 8 of Northern Louisiana, including the specified parishes.
• Active, unrestricted Louisiana Registered Nurse license required.
• At least two years of clinical experience in maternal-child health, Labor & Delivery, and/or pediatrics.
• Intermediate to advanced computer skills, including Microsoft Word, Outlook, Excel, and electronic medical records.
• Strong communication, interpersonal, independent judgment, and care-plan adaptation skills.
• Availability for Monday–Friday, 8:00 AM–5:00 PM Central Time, with up to 50% regional travel required.

Nice to have

• Case Management Certification or willingness to obtain it within two years.
• Field case management or managed care health plan experience.
• Experience with Medicare, Medicaid, and dual-eligible populations.
• Knowledge of breastfeeding, maternal-child mortality and morbidity, community health resources, health promotion, coaching, wellness, or motivational interviewing.
