Case Manager (Healthcare)

Location: Remote Flex - North Carolina, United States. Remote work is supported with a few required in-office visits each year; the exact onsite expectations depend on the role and distance from the Durham headquarters.

Salary: $73,698.00–$117,917.00 per year, plus potential annual incentive bonus.

What you will do

• Coordinate care, resources, and services for members as part of a multidisciplinary team.
• Perform comprehensive clinical assessments and develop member-centered treatment, intervention, and discharge plans.
• Monitor member progress, conduct follow-ups, provide education, and adjust goals or services based on changing needs.
• Coordinate care across physician offices, hospitals, rehabilitation units, skilled nursing facilities, and home care.
• Evaluate medical necessity, treatment alternatives, benefits, and available community or funding resources.
• Document assessments, care plans, interventions, monitoring activities, and outcomes; develop policies or training when needed.

Requirements

• Registered Nurse with at least 3 years of clinical and/or case management experience.
• Valid and maintained North Carolina or compact multistate clinical licensure is required.
• Additional specialty certification, such as CCM or CDCES, may be required for certain roles and must be obtained within 2 years when applicable.
• Ability to collaborate with members, families, health care providers, internal partners, and external services.

Nice to have

• Additional non-compact state nursing licenses.
• Broad clinical nursing experience across medical-surgical, ICU, emergency, pediatrics, labor and delivery, or other inpatient settings.
