Location: Mauston, Wisconsin and the surrounding Juneau County area; remote work with up to 40% local travel and in-person member visits

Salary: $53,700–$72,600 per year

What you will do

• Assess members’ health, safety, strengths, interests, and preferences.
• Develop and update comprehensive Member Care Plans.
• Coordinate with Field Care Nurses and arrange health, social, housing, employment, and community support services.
• Conduct face-to-face assessments at enrollment, recurring quarterly in-person visits, and monthly phone contacts.
• Evaluate risks, educate members, and support cost-effective service delivery.
• Maintain case notes, service authorizations, and care plan documentation.

Requirements

• Bachelor’s degree in human services or a related field plus at least one year of experience serving frail elders or adults with intellectual, developmental, or physical disabilities; alternatively, a bachelor’s degree in another field plus at least three years of relevant experience.
• Valid state driver’s license and personal vehicle liability insurance with 100/300/100 limits are required.
• Ability to work in the Mauston, Wisconsin area and travel primarily within Juneau County and surrounding areas.
• Availability for Monday–Friday, 8:00 a.m.–4:30 p.m. CST work hours.
• Ability to use a dedicated, interruption-free workspace and internet service with at least 25 Mbps download and 10 Mbps upload speeds.

• Residence within 45 minutes of the assigned coverage area.
• Case management experience.
• Experience with electronic case note documentation.
• Knowledge of community health, social service agencies, and local resources.
