Case Manager - Hayward

Location: Remote Wisconsin, supporting members in the Hayward/Park Falls, WI area; travel up to 40% with recurring face-to-face member visits and occasional travel to hirify.global offices.

Salary: $53,700–$72,600 per year

What you will do

• Assess member health, safety, strengths, interests, and preferences to develop comprehensive Member Care Plans.
• Coordinate with Field Care Nurses and arrange health, social, housing, employment, and community support services.
• Conduct enrollment and six-month social assessments, quarterly in-person visits, and monthly phone contacts.
• Evaluate risks, educate members, and support cost-effective service delivery in the least restrictive environment.
• Maintain case notes, service authorizations, and Member Care Plan updates.

Requirements

• Bachelor’s degree in human services or a related field plus at least one year of experience serving frail elders or adults with intellectual, developmental, or physical disabilities; alternatively, a bachelor’s degree in another field plus at least three years of relevant experience.
• Valid state driver’s license and personal vehicle liability insurance with minimum 100/300/100 limits.
• Ability to work Monday–Friday, 8:00 am–4:30 pm CST and travel up to 40% in the assigned coverage area.
• Ability to reside within 45 minutes of the assigned coverage area is preferred.
• Patient-facing work requires participation in hirify.global’s TB screening program.

Nice to have

• Case management experience.
• Experience with electronic case note documentation.
• Knowledge of community health, social service agencies, and other community resources.
