Case Manager - Richland County

Location: Remote Wisconsin, supporting members in the Richland County and Iowa County areas. Up to 40% travel is required, including recurring in-person visits at members' residences and occasional travel to hirify.global offices. Typical hours are Monday–Friday, 8:00 a.m.–4:30 p.m. CST.

Salary: $53,700–$72,600 per year

What you will do

• Assess members' health, safety, strengths, interests, and preferences.
• Develop and update comprehensive Member Care Plans.
• Coordinate services with Field Care Nurses and community providers.
• Conduct enrollment and follow-up social assessments, quarterly in-person visits, and monthly phone contact.
• Arrange social integration, community, employment, housing, and other non-medical support services.
• Evaluate risks, educate members, manage service authorizations, and maintain accurate case documentation.

Requirements

• Bachelor's degree in human services or a related field plus at least one year of experience serving frail elders or adults with intellectual, developmental, or physical disabilities; alternatively, a bachelor's degree in another field plus at least three years of relevant experience.
• Valid state driver's license and personal vehicle liability insurance with minimum 100/300/100 limits.
• Ability to work in the Richland County and Iowa County coverage area and complete required travel.
• Ability to work from a dedicated space with internet speeds of at least 25 Mbps download and 10 Mbps upload while protecting member PHI and HIPAA information.
• Completion of TB screening is required for this patient-facing role.
