RN Home Health Case Manager (Lowell Area)

Location: Lowell, Massachusetts and surrounding areas; home visits required. Full-time daytime schedule with rotating evening, night, weekend, and holiday coverage.

Salary: $84,300.22–$107,481.20 annually based on full-time equivalent.

What you will do

• Assess patients’ physical, psychosocial, educational, and supportive care needs in their homes.
• Develop and evaluate individualized treatment plans with patients, families, physicians, community resources, and interdisciplinary teams.
• Manage an assigned caseload, prioritize visits and calls, provide telephone triage, and help prevent avoidable hospitalizations.
• Complete physicians’ orders, OASIS assessments, medication reconciliation, and timely clinical documentation.
• Coordinate care conferences, discharge planning, patient transitions, and responses to critical needs or crises.
• Provide safe care while working independently in homes and responding appropriately to safety and emergency situations.

Requirements

• Active RN license in the state where services are provided; any additional required state license must be obtained within 60 days.
• Valid state-issued driver’s license and one year of acute medical/surgical nursing experience.
• Basic Life Support certification; CPR is required upon hire for candidates hired before September 28, 2026, while BLS must be obtained within 30 days for candidates hired after that date.
• Ability to communicate, read, and write in English; strong assessment, organization, prioritization, and time-management skills.
• Ability to work autonomously in a home care setting and regularly lift 25 pounds, with occasional lifting of up to 100 pounds.

Nice to have

• Bachelor of Science in Nursing and specialty nursing certifications.
• Three years of acute nursing experience, including medical/surgical, oncology, hospice, or home health.
• Experience with IV therapy and complex wound care.
