Case Manager (Healthcare)

Location: Remote within the Green Lake, Marquette, Adams, and Waushara County coverage area in Wisconsin, United States. The role includes face-to-face member visits, up to 40% travel, and occasional travel to hirify.global offices for training or meetings. Typical hours are Monday–Friday, 8:00 a.m.–4:30 p.m. CST.

Salary: $53,700–$72,600 per year.

What you will do

• Assess members’ health, safety, strengths, interests, and preferences and develop comprehensive Member Care Plans.
• Coordinate with Field Care Nurses and arrange services in the least restrictive environment.
• Conduct enrollment and recurring face-to-face social assessments, quarterly in-person visits, and monthly phone contacts.
• Connect members with social integration, community, employment, housing, and other non-medical resources.
• Evaluate risks, provide member education, and support cost-effective service delivery.
• Maintain case notes, service authorizations, care plan updates, and other documentation.

Requirements

• Bachelor’s degree in human services or a related field plus at least one year of experience serving frail elders or adults with intellectual, developmental, or physical disabilities; alternatively, a bachelor’s degree in another field plus at least three years of relevant experience.
• Valid state driver’s license and personal vehicle liability insurance with minimum 100/300/100 limits.
• Ability to conduct in-person visits and travel up to 40% across the assigned Wisconsin counties.
• Ability to work Monday–Friday during CST business hours.
• For remote work, dedicated workspace and internet service with at least 25 Mbps download and 10 Mbps upload are required to protect member PHI and HIPAA information.
• Successful candidates must complete tuberculosis screening.

Nice to have

• Residence within 45 minutes of the assigned coverage area.

• Experience with electronic case note documentation and knowledge of community health and social service agencies.
