Care Manager, RN

Location: Remote in Kansas, with preference for candidates near the northwest service area; approximately 50% regional travel and home visits are required.

Salary: $56,200–$101,000 per year

What you will do

• Enroll Medicaid members in care management and assess their physical, social, and healthcare needs.
• Develop, update, and monitor personalized care plans and service plans.
• Coordinate with members, families, caregivers, providers, specialists, and community resources.
• Identify barriers to care and facilitate access to appropriate services, referrals, and benefits.
• Provide telephonic, digital, home, and site-based outreach, including member and family education.
• Document care activities and member information in compliance with state, federal, and payer requirements.

Requirements

• Must live in Kansas, preferably near the northwest service area, including Cheyenne, Greenlee, Barton, Smith, and surrounding counties.
• Approximately 50% travel is required, including driving throughout the region and completing home visits based on member needs.
• Degree from an accredited school of nursing or a Bachelor’s degree in Nursing.
• Two to four years of related experience.
• RN state licensure and/or compact-state licensure is required.
