Case Manager - Luck/Rice Lake (Healthcare)

Location: Remote position serving the Rice Lake/Luck, Wisconsin area; quarterly in-person visits and face-to-face assessments are required, with up to 40% travel. Standard schedule is Monday–Friday, 8:00 am–4:30 pm CST.

Salary: $53,700–$72,600 per year

What you will do

• Assess members’ health, safety needs, strengths, interests, and preferences.
• Develop and update comprehensive Member Care Plans.
• Coordinate with Field Care Nurses and arrange services in the least restrictive environment.
• Conduct enrollment and recurring face-to-face assessments, quarterly in-person visits, and monthly phone contact.
• Connect members with social integration, community, employment, housing, and other non-medical resources.
• Evaluate risks, provide education, manage service authorizations, and maintain case documentation.

Requirements

• Bachelor’s degree in human services or a related field plus at least one year of experience serving frail elders or adults with intellectual, developmental, or physical disabilities; alternatively, a bachelor’s degree in another field plus at least three years of relevant experience.
• Valid state driver’s license and personal vehicle liability insurance with minimum 100/300/100 limits.
• Ability to conduct patient-facing, in-person visits and travel up to 40% within the assigned coverage area.
• Ability to work Monday–Friday during CST business hours.
• Residence within 45 minutes of the assigned coverage area is preferred.
• Ability to work securely from a dedicated home workspace protecting member PHI and HIPAA information; internet service must provide at least 25 Mbps download and 10 Mbps upload speeds.
