Care Manager (Behavioral Health)

Location: Remote in Camden or Gloucester County, New Jersey, or a nearby bordering county; up to 50% local travel required for member and site visits. The closest company office is in Iselin, New Jersey. Schedule: Monday through Friday, 9:00 AM to 5:00 PM.

Salary: $56,200–$101,000 per year

What you will do

• Assess member needs through phone, digital, in-home, and other site visits, including behavioral health and social determinant needs.
• Develop and monitor care plans for members with high-acuity mental health and substance use disorder needs.
• Coordinate members, families, caregivers, providers, specialists, community resources, and care teams to ensure timely access to services.
• Track treatment side effects, complications, clinical symptoms, and member outcomes, recommending updates to care plans.
• Educate members and families about treatment options, referrals, healthcare benefits, and provider instructions.
• Document care management activities and provide feedback on opportunities to improve quality and cost-effective care delivery.

Requirements

• Master’s degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing.
• 2–4 years of related experience and behavioral health experience.
• Licensed Master’s Behavioral Health Professional, such as LCSW, LMSW, LMFT, LMHC, or LPC, or an RN based on state contract requirements.
• For the New Jersey Health Plan: Master’s degree in Behavioral Health or Social Work plus an active New Jersey LAC or LSW license.
• Residence in Camden or Gloucester County, New Jersey, or a nearby bordering county is required.
