Virtual Seasonal Advanced Practice Provider (APP) (NP/PA) (Telehealth)

Location: Remote USA; primarily telehealth video visits with limited occasional in-home visits for members in the local area.

Base compensation: $60,000 flat pay for the 5-month assignment. Performance bonus: up to $5,000; total target compensation up to $65,000.

What you will do

• Conduct primarily video-based telehealth visits and limited occasional in-home visits.
• Perform comprehensive assessments and provide evidence-based preventive care, chronic condition support, medication optimization, and health maintenance.
• Identify care gaps and coordinate with primary care providers, specialists, pharmacists, nurses, caregivers, and families.
• Contribute to individualized, whole-person care plans for Medicare members.
• Use integrated technology and AI-enabled workflows to reduce administrative work and improve member interaction.
• Communicate clearly and empathetically with members and caregivers.

Requirements

• Active and clear Nurse Practitioner (APRN/NP) or Physician Assistant (PA) license required.
• At least 3 years of outpatient clinical practice, preferably in primary care, family medicine, internal medicine, or geriatrics.
• Willingness to obtain additional licenses within the first 90 days when necessary; support and reimbursement are provided.
• Active BLS certification at the time of hire.
• Comfort delivering care through video telehealth and using electronic documentation systems.
• Ability to work a guaranteed full-time schedule of 40 hours per week from November through March 31.

Nice to have

• Familiarity with managed care models, including STARS/HEDIS and care-gap identification.
• Experience with comprehensive or preventive care visits for Medicare populations.
• Experience supporting older adults or people with complex medical or social needs.
