Virtual Physician – Transitions of Care (Hospitalist / Post-Acute) (AI)

Location: Remote USA

Salary: $280,000–$338,000 per year, plus eligibility for a discretionary performance-based annual bonus with a 10% target.

What you will do

• Deliver virtual transitions-of-care services to recently discharged patients at high risk of readmission during the 30-day post-discharge period.
• Act as the final clinical escalation point for complex patient management decisions and oversee key clinical outcome metrics.
• Supervise, teach, and coach nurse practitioners through collaborative practice agreements across multiple states.
• Lead weekly interdisciplinary rounds and readmission conferences, including deep-dive root cause analyses.
• Collaborate with physicians, nurse practitioners, nurses, medical assistants, clinical coordinators, operations leads, and senior managers.
• Drive quality improvement, care equity, process innovation, and the development and use of AI-enabled clinical tools.

Requirements

• MD or DO degree and board certification in Internal Medicine, Family Medicine, or a related specialty.
• At least 3 years of post-residency clinical experience with a strong background in inpatient care and transitions of care.
• Experience managing chronic and acute conditions including heart failure, COPD, diabetes, hypertension, pneumonia, cellulitis, and urinary tract infections.
• Experience leading interdisciplinary rounds, conducting root cause analyses, and supervising nurse practitioners or other healthcare professionals.
• Strong analytical, problem-solving, change-management, collaboration, and clinical innovation skills.
• Active medical licensure in good standing, with the ability and willingness to obtain additional state licenses as the program expands.

Nice to have

• Experience in hospital medicine, post-acute care, primary care, geriatrics, value-based care, or Medicare Advantage.
• Experience as a medical director or in a similar physician leadership role.
• Experience delivering remote or virtual care and using telehealth platforms.
• Background in quality improvement methodologies and clinical initiative leadership.
• Experience working in high-growth healthcare organizations and serving diverse patient populations.
