Case Manager - Ashland/Superior

Location: Remote position supporting the Ashland/Superior area in Wisconsin; preferred residence within 45 minutes of the assigned coverage area. Up to 40% travel and occasional travel to hirify.global offices for training or meetings.

Salary: $53,700–$72,600 per year

What you will do

• Assess members’ health, safety needs, strengths, interests, and preferences to develop comprehensive Member Care Plans.
• Coordinate with Field Care Nurses to provide services in the least restrictive environment.
• Conduct face-to-face social assessments at enrollment and at least every six months, typically at members’ residences.
• Complete quarterly in-person visits and maintain monthly phone contact with members.
• Arrange social integration, community, employment, housing, and other non-medical support services.
• Evaluate risks, educate members, manage service authorizations, and maintain accurate case documentation.

Requirements

• Bachelor’s degree in human services or a related field plus at least one year of experience serving frail elders or adults with intellectual, developmental, or physical disabilities; alternatively, a bachelor’s degree in another field plus at least three years of relevant experience.
• Valid state driver’s license and personal vehicle liability insurance with minimum limits of 100/300/100.
• Ability to support members in the Ashland/Superior area, travel up to 40%, and work Monday–Friday from 8:00 a.m. to 4:30 p.m. CST.
• Ability to work from a dedicated home workspace protecting member PHI and HIPAA information.
• Home internet with at least 25 Mbps download and 10 Mbps upload speeds.
