Location: Remote in Illinois, United States; occasional travel to offices for training or meetings may be required. Working hours are Monday–Friday, 8:00am–5:00pm CST, with occasional scheduling flexibility.
Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do
- Conduct phone-based health assessments for an assigned Illinois member population.
- Create and maintain care plans, provide chronic-condition education, and address gaps in care.
- Identify and resolve physical, environmental, and psychosocial barriers to effective care.
- Monitor patient progress toward desired outcomes through assessments, conversations, data, and active care planning.
- Navigate multiple systems and apply critical-thinking skills to coordinate member support.
Requirements
- Bachelor’s degree in nursing or a related health field.
- Active Illinois Registered Nurse licensure with no disciplinary action, or the ability to obtain Illinois RN licensure.
- At least three years of clinical acute-care experience as a licensed RN.
- Experience with health promotion, coaching, and wellness.
- Ability to work remotely from Illinois and maintain a dedicated workspace protecting member PHI and HIPAA information.
- Home internet with at least 25 Mbps download and 10 Mbps upload speed.
Nice to have
- Bilingual English/Spanish, English/Polish, or English/Creole skills.
- Experience with case management, discharge planning, and adult acute-care patient education.
- Managed-care experience or experience supporting Medicare and Medicaid recipients.
- Certified Case Manager certification.
- Knowledge of community health, social-service agencies, and community resources.
