Care Coordinator (Healthcare)

Location: Remote within Arizona, Florida, Georgia, or Texas, United States. Occasional travel may be required for team meetings and company events.

Salary: $17.91–$23.51 per year.

What you will do

• Act as the primary bridge and entry point for members and providers working with the Care Management team.
• Handle inbound calls through optimized IVR pathways and work toward one-call resolution.
• Coordinate care for members in Complex Care Management, transition-of-care, readmission prevention, and unassigned populations.
• Arrange non-emergency medical transportation through UberHealth while following program requirements.
• Resolve internal escalations to Claims, Eligibility, Care Navigation, and other specialized departments.
• Lead special projects and collaborate across departments to improve the member experience.

Requirements

• 1+ year of experience in care coordination and navigation in a healthcare setting.
• Strong computer literacy and aptitude for electronic medical records such as Medecision.
• Ability to use telephony systems such as InContact.
• Excellent interpersonal communication and listening skills.
• Residence in Arizona, Florida, Georgia, or Texas is required for this remote role.

Nice to have

• Medical Assistant certification.
• Bilingual communication skills.
• Familiarity with health insurance.
