What you will do

• Enroll patients in the palliative care program and facilitate connections with providers.
• Educate patients and caregivers on the value and specifics of the care program.
• Document patient encounters and triage needs to the appropriate care team.
• Act as a primary point of contact for patients, families, and providers.
• Follow up with patients to ensure high-quality care delivery.
• Collaborate with the Central Operations team on process improvements.

Requirements

• Must be based in the United States
• Minimum 2+ years of relevant experience in a customer service call center or healthcare setting.
• Basic understanding of the healthcare industry.
• Strong communication skills with an empathetic and compassionate approach.
• Comfortable working with technology in a fast-paced startup environment.
• Reliable and secure Wi-Fi connectivity.
