Care Coordinator (Fully Remote) (Healthcare)

Salary: $37,440–$41,600 per year

hirify.global provides palliative and hospice care for seniors with serious illness, working with payers, risk-bearing entities, and community care partners to improve outcomes and reduce over-medicalized end-of-life care.

What you will do

• Help patients understand the value of palliative care and enroll them in the program.
• Connect patients with palliative care providers and answer questions from patients, caregivers, and providers.
• Document patient encounters and triage patient needs to the appropriate care team.
• Follow up with patients to ensure they receive high-quality care from the provider network.
• Collaborate on process improvement within Central Operations.

Requirements

• Must be based in the United States and work remotely.
• At least 2 years of relevant experience in a customer service call center or healthcare environment.
• Basic understanding of healthcare and strong communication skills.
• Empathetic, compassionate, well-spoken, and comfortable speaking with patients by phone.
• Comfort with technology, a dynamic startup environment, and secure Wi-Fi connectivity.

Nice to have

• Background in social work or another people-centered field.
• Bilingual Spanish and English communication skills.
