Case Manager (Healthcare)

Location: Remote Flex - North Carolina; virtual work with a few in-office visits each year, with onsite expectations depending on distance from the Durham headquarters.

Salary: $73,698–$117,917 annually, plus an annual incentive bonus.

What you will do

• Coordinate care, resources, and services for selected member populations as part of a multidisciplinary team.
• Perform comprehensive assessments and develop member-centric treatment, intervention, and discharge plans.
• Monitor members’ progress, arrange referrals and education, and modify care plans according to acuity and needs.
• Evaluate and facilitate care across physician offices, hospitals, rehabilitation units, skilled nursing facilities, and home care.
• Assess medical necessity, service appropriateness, available benefits, and potential benefit exceptions.
• Document assessments, care plans, interventions, monitoring activities, and outcomes.

Requirements

• Registered Nurse with at least 3 years of clinical and/or case management experience.
• Valid and applicable North Carolina or compact multistate clinical licensure is required and must be maintained.
• Additional specialty certification, such as CCM or CDCES, may be required for some roles and must be obtained within two years when applicable.
• Experience with care planning, clinical outcomes, healthcare utilization management, patient evaluations, and patient care coordination.

Nice to have

• Additional non-compact state nursing licenses alongside a compact multistate license.
• Broad clinical nursing experience across medical-surgical, ICU, emergency, pediatrics, labor and delivery, or other inpatient specialties.
