Government Programs Care Manager III - Work From Home

Location: Austin, Texas, with field sites in the Bryan/College Station area; regular travel within the assigned territory and visits to members’ homes or facilities may be required

Base pay: $26.90–$59.35

What you will do

• Provide telephonic and field-based care coordination for government program members.
• Educate members about healthcare programs and support adherence to personal health goals.
• Conduct health screenings, comprehensive assessments, and care planning for complex and specialty populations.
• Respond to member inquiries and coordinate activities with providers, internal teams, vendors, and community resources.
• Support members through visits to homes or facilities when required.

Requirements

• Hold an unrestricted RN license with at least two years of direct clinical care experience, or an applicable unrestricted behavioral health license with two years of clinical practice.
• Maintain a current, valid, unrestricted license in the state of operations or qualify through reciprocity; compact RN licensees changing permanent residence must obtain state licensure within 90 days of hire.
• Have at least three years of wellness or managed care experience presenting clinical issues to members and physicians.
• Demonstrate knowledge of health and wellness markets, employer trends, communication, medical data analysis, databases, and web-based applications.
• Hold a current unrestricted driver’s license, transportation, and applicable insurance; ability and willingness to travel within the assigned territory is required.
• Texas candidates must have experience working with children and young adults with behavioral health issues in three of the last five years.

Nice to have

• Three years of direct clinical experience and patient education experience.
• Condition management, complex or catastrophic case management, government programs, or population management experience.
• Bilingual English and Spanish communication skills.
• Case management, training, project management, or nationally recognized healthcare certification.
