TL;DR
LTSS Service Care Coordinator (Long-Term Services and Supports): Coordinating holistic care management, long-term care plans, provider services, and community resources for Illinois Health Plan members with physical disabilities and aging-related needs, with an accent on member assessments, service planning, and in-home visits. Focus on evaluating member needs, coordinating caregivers and healthcare providers, monitoring care outcomes, and facilitating access to benefits and support services.

Location: Chicago, Illinois, primarily serving specified Chicago-area ZIP codes; hybrid work with approximately 75% local travel for in-home member visits. Training is Monday-Friday, 8:00 AM-5:00 PM CT; the regular schedule is Monday-Friday, 8:00 AM-4:30 PM CT.

Salary: $22.94-$38.79 per hour.

What you will do

  • Evaluate member needs, available resources, and appropriate care outcomes.
  • Develop and monitor long-term care plans and service plans for members receiving LTSS.
  • Coordinate services between members, families, caregivers, healthcare providers, specialists, and community resources.
  • Provide resource support related to housing, employment, independent living, participant direction, justice, and foster care.
  • Educate members and caregivers about service options, referrals, provider instructions, procedures, and healthcare benefits.
  • Document care management activities, monitor outcomes, support regulatory compliance, and recommend improvements to care delivery.

Requirements

  • Bachelor’s degree and at least one year of related experience.
  • Must reside in the Chicago-area service territory or an approved nearby ZIP code and be able to perform extensive local travel and in-home visits.
  • For the Illinois Physically Disabled/Elderly plan, meet one of the following: Illinois RN license; a bachelor’s or master’s degree in a qualifying human services-related field; or an LPN license with one year of experience conducting comprehensive assessments and providing formal services for elderly people.
  • Experience in case management, advocacy, or home visits is strongly preferred.
