Location: Seattle, Washington, United States; on-site role with in-home visits and statewide travel across Oregon

Salary: Approximately $140,000 annually, plus sign-on bonus

What you will do

  • Travel to members’ homes and conduct comprehensive health evaluations and Annual Wellness Visits.
  • Perform medical histories, physical examinations, reviews of systems, vital-sign collection, medication reconciliation, and preventive screenings.
  • Conduct or support screenings and tests including Hemoglobin A1C, spirometry, KED, DRE, FIT, and bone-density evaluations.
  • Educate patients on preventive care, medication adherence, chronic-condition management, goals of care, and informed decision-making.
  • Identify diagnoses for care management and risk-adjustment documentation, close quality-care gaps, and communicate findings to primary care providers.
  • Maintain compliant EMR documentation and protect patient health information under HIPAA.

Requirements

  • Active, unencumbered Nurse Practitioner license in Oregon is required.
  • At least three years of direct patient-care experience; primary care or adult/geriatric experience is preferred.
  • Ability to support the entire state of Oregon, with daily travel within a 55-mile radius of the assigned home location.
  • Statewide travel may be scheduled with at least two weeks’ notice.
  • EMR proficiency is desired.
  • Bilingual proficiency is advantageous.

  • Prior in-home risk-assessment experience.
  • Primary care or adult/geriatric clinical background.
