Nurse Practitioner (Remote, CT License Required)

Location: Remote, United States; active CT license required

Compensation: $75/hour regular rate and $85/hour premium rate.

What you will do

• Provide phone- and video-based follow-up care for patients recently discharged from hospitals.
• Conduct telephonic outreach and scheduled video or phone visits.
• Perform medication reconciliation and symptom screening, and order follow-up labs, medications, or supplies.
• Build patient trust, explain the care model, and connect patients with appropriate services.
• Coordinate care across hospitals, emergency departments, pharmacies, and community providers.
• Collaborate with nursing and administrative colleagues while contributing to individual and population health goals.

Requirements

• Active Nurse Practitioner license in Connecticut required.
• Certification as an Adult, Family, or Geriatric Nurse Practitioner.
• At least 5 years of post-graduate clinical experience in primary care or geriatrics; up to 2 years may be substituted with skilled-nursing or long-term-care experience.
• Experience caring for Medicare and Medicaid populations with complex, comorbid conditions.
• Strong motivational interviewing, communication, collaboration, problem-solving, and digital technology skills.
• Availability for at least 24 hours per week, including monthly weekend shifts across 4-hour weekday or weekend shifts.

Nice to have

• Active licenses in NJ, TX, PA, IL, NY, MA, MN, LA, or SC.
• Familiarity with Google Suite and Slack.
• Bilingual English and Spanish communication skills with cultural fluency.
